False PHD is a passive Devil Room item introduced in the Repentance DLC of The Binding of Isaac: Rebirth. It functions as a risky, high-reward alternative to the standard PhD item, identifying all pills in your run while twisting their core properties to grant massive damage boosts or health generation. Pill Identification: Automatically identifies all pills upon pickup.
Immediate Health: Spawns one Black Heart and a random pill when you first pick it up.
Pill Corruption: Converts all “good” stat pills into their negative, worse counterparts (e.g., Tears Up becomes Tears Down).
The Damage Trade-off: Taking any Stat Down pill rewards you with a permanent +0.6 Damage Up (+1.2 for Horse Pills). This effect is retroactive; you immediately gain damage for any bad pills you swallowed before grabbing the item.
Black Heart Generation: Swallowing a non-stat negative pill (such as Amnesia, R U A Wizard?, or Paralysis) rewards you with a Black Heart. Notable Item Synergies
Rock Bottom: This is the ultimate pairing. It locks your stats at their highest point, allowing you to consume bad pills for the free damage upgrade without actually losing your other stats.
Regular PHD / Lucky Foot / Virgo: Mixing False PHD with a positive pill item results in a neutral status where pills can be both good and bad again. However, taking a negative pill still triggers False PHD’s damage or Black Heart bonus.
Placebo: Allows you to repeatedly use the same pill. If you find a relatively harmless Stat Down pill (like Luck Down or Shot Speed Down), you can repeat it to generate massive amounts of damage.
